For referral partners
Discharges that hold
We work alongside hospitals, physicians, case managers, social workers, MCOs and discharge planners to make the transition from hospital to home seamless, and to keep children safely at home once they’re there.
What you can expect
How we work with you
A straight answer
Whether we can cover the case, and by when. Declined quickly beats accepted vaguely.
One point of contact
You are not re-explaining the case to a different person each time you get in touch.
Documentation you can use
Nursing notes written so a physician or reviewer can act on them.
Matching that sticks
Fewer restarts, because we match on family fit as well as clinical fit.
Who we take referrals from
Hospitals, physicians, case managers, MCOs
We serve families discharging from hospitals across Greater Philadelphia, and take referrals from physicians, case managers, social workers, managed care organizations and payers, as well as directly from families.
- Timely communication at every step
- Seamless hospital-to-home transitions
- Clinical expertise in medically complex pediatrics
- Nurse matching that reduces refusals and restarts

Questions
Questions from referring professionals
What do you need to accept a referral?
The child’s clinical needs, the ordered hours, the payer and authorization status, the anticipated discharge date, and a contact for the case manager. If something is missing we will still take the enquiry, but we cannot confirm until we have it.
How quickly will we hear back?
We will acknowledge the referral and tell you what we can and cannot cover. We would rather decline within a day than hold a case open and leave you looking for an alternative at the last minute.
Will you tell us if you cannot staff it?
Yes, and quickly. A referral accepted and not staffed costs a discharge planner more than a referral declined.
Have a discharge coming up?
Get in touch before the date is set. It is the single cheapest thing anyone can do to protect it.
